Information and referral services are the first step towards answering your questions or helping you solve problems. Perhaps you need assistance in locating services for an older relative. We can assist you with completion of forms for Medicaid, Energy Assistance, Prescription Medications, Golden Buckeye Cards, Medicare, Medicaid programs, Heap Forms, Social Security Forms and more. We can provide information and make referrals to other resources if information is needed on a service we do not provide.

Additionally, we provide Health Fairs and speakers for groups as requested.
